What information do I need to provide?

When calling Alliance Medical Transport, be prepared to give the communications coordinators the following information:

  • Your name and the requesting agency
  • A complete patient report
  • A call-back phone number and contact name
  • Weather conditions
  • For inter-hospital transfer of patients, include the names of the hospitals and the receiving physician.

Necessary Transport Paperwork Checklist

To transfer a patient between hospitals, COBRA, Medicare and several other agencies require specific paperwork. The information to be sent with the patient includes:

  • Two copies of the hospital face sheet
  • The history and physical cardiac cath lab reports, if applicable
  • A physician transfer order, signed by the physician
  • Physician orders, also signed by the physician
  • Physician certification statement
  • Please advise whether CINE films or CD's are to be sent with the patient, or if they have already been sent.

University Air Care

When calling Air Care, be prepared to give the communications specialist the following information:

  • Your name and the requesting agency
  • A brief description of the incident and number of people injured or ill
  • A call-back phone number, name, and if possible, a radio frequency the helicopter can use for air-to-ground communications at the scene
  • Helipad DiagramThe location of the nearest 100-foot-square landing site free of overhead obstructions. Be sure to include cross streets and landmarks. At night, mark the area with flares or lights. Take special care to avoid shining lights directly at the helicopter.
  • Weather conditions
  • For inter-hospital transfer of patients, include the names of the hospitals and the receiving physician

What is the response time?

Mobile Care

Alliance Mobile Care stages vehicles throughout the city to ensure the fastest response time possible. Our communications coordinators work closely with referring and receiving facilities to ensure all needs are met.

University Air Care

University Air Care has a helicopter based at The University Hospital and at University Pointe in Butler County to ensure the fastest response time.

University Air Care can make a round trip within 150 miles of Cincinnati. Longer flights usually require fixed-wing aircraft, which the University Air Care Communications Center can arrange. View our Flight Range map.
